Dietitian Nutritionist

The Dietitian Nutritionist is responsible of educating and delivering medical nutrition therapy interventions/orientations according to patient’s needs. This professional works with an interdisciplinary care team in order to accomplish clinical and operational metrics of the program.


  • Design and facilitate educations and interventions to promote patient wellness
  • Maintain direct contact with patients through telephone calls; perform follow ups and telephone consultations regarding patient needs referred by the interdisciplinary care team
  • Offer coaching interventions and nutritional educations/orientations in order to achieve patient health goals
  • Prepare key performance indicators reports
  • Realize home visits and perform a nutritional assessment to implement the medical nutrition therapy required according to patient needs
  • Among other tasks as required by the program


  • Bachelor Degree in Nutrition and Dietetics
  • Current Unrestricted License in Nutrition and Dietetics (LND)
  • Diabetes Educator (CDE); preferable
  • Registered Dietitian Nutritionist (RDN); preferable
  • Minimum of two (2) years clinical experience in health industry withing nutritional scenarios
  • Intermediate computer skills
  • Bilingual (English and Spanish); preferable
  • Excellent communication skills (spoken, written and reading)
