Lifestyle Coach

Responsible for completing a health questionnaire (HRA) to members of the Advantage population. This questionnaire is created in order to identify the lifestyles and level of health risk of medicare beneficiaries. With these responses, a comprehensive plan can be created to assist the beneficiary in health care.

  • Bachelor in Social Work, Psychology or Nursing;
  • Minimum of one year of experience in Customer Service and / or scenarios related to health;
  • Experience in managing computerized systems;
  • Orientation and commitment to customer service;
  • Excellent verbal and written communication skills
  • Mobile Phone, Computer / Laptop and Internet connection (20 MB minimum)
